We divide our groups into teams.  Whenever possible, we try to do this by age, and to honor requests if you have a friend or sibling you would like your child to be with.  If there is a friend your child would like to be placed with, please list their name here.  We will do our best to honor that request. 


I hereby release First Baptist Church Hopewell from all liabilities should my child be injured at any church sponsored program.  I understand that my child must be signed in and out each day by an adult, and that my child must be picked up on time at the conclusion of activities.  I have provided the camp staff with an emergency contact number where I or another adult may be reached.  Should my child be injured, I give FBC Hopewell permission to seek treatment for my child.  Additionally, I authorize FBC Hopewell to use pictures and or video clips that include by child for the promotion of future events .
